Dr. Nicole Biediger
Board-Eligible General Surgeon, DO
Dr. Nicole Biediger is a board-eligible general surgeon dedicated to providing compassionate, patient-centered surgical care. She earned her Doctor of Osteopathic Medicine degree from the University of the Incarnate Word School of Osteopathic Medicine in San Antonio before completing her General Surgery residency at Texas Tech University Health Sciences Center El Paso, where she trained at a high-volume Level I Trauma Center and gained extensive experience in robotic, laparoscopic, and open surgery.
Dr. Biediger treats a broad range of surgical conditions, including hernias, gallbladder disease, soft tissue masses, skin lesions, and gastrointestinal disorders, with a particular interest in minimally invasive and robotic techniques that promote faster recovery and improved patient outcomes. She is committed to building strong relationships with her patients through clear communication, education, and shared decision-making, ensuring each patient feels informed and supported throughout their surgical journey.
Outside of the operating room, Dr. Biediger enjoys traveling, staying active, baking sourdough, and spending time with family and friends.

Dr. Rex Farrer
Bariatric & General Surgeon · Master of Bioethics
Dr. Farrer specializes in the surgical treatment of obesity and its associated medical issues to promote weight loss and improve overall health. He focuses on surgical weight loss procedures including gastric sleeve, gastric bypass, and single anastomosis duodenal switch. Dr. Farrer also performs revisional weight loss procedures, gallbladder removal, and the surgical management of heartburn and acid reflux.
Dr. Farrer completed a Master of Bioethics degree and attended Kansas City University of Medicine and Biosciences in Kansas City, MO for his medical education. He completed an internship at Mercy St Vincent Medical Center in Toledo, OH, and his General Surgery Residency at Mercy St. Vincent Medical Center. Dr. Farrer also completed a fellowship in Minimally Invasive Surgery and Bariatric Surgery at Promedica/University of Toledo in Toledo, OH.
Dr. Farrer uses the most state-of-the-art surgical techniques and procedures, including the DaVinci Robot. He performs surgeries utilizing both laparoscopic and robotic approaches. Dr. Farrer is an active member of the American Society for Metabolic and Bariatric Surgery (ASMBS), the American College of Osteopathic Surgeons (ACOS), and the Texas Medical Association.
When he is not seeing patients or performing surgery, Dr. Farrer enjoys spending time with his wife and five children. Dr. Farrer spends evenings and weekends running his kids to soccer, dance, and theater and engaging in church activities.

Dr. Lydia C. Piper
Board-Certified General Surgeon · MD, FACS
Joining the practice in October 2026.
Dr. Piper is a board-certified General Surgeon and Fellow of the American College of Surgeons with interests in robotic hernia surgery, breast surgical care, and colorectal disease.
A native of Richmond, Virginia, she completed her medical training at Johns Hopkins University School of Medicine in Baltimore, Maryland. Upon graduation, she was commissioned as an officer in the United States Army and went on to complete her residency in General Surgery at Brooke Army Medical Center in San Antonio.
She then served as a General Surgeon in Germany, caring for service members and their families stationed overseas. During her time in the military, she worked to coordinate efforts ensuring access to breast cancer and robotic surgical care for US military families in Europe, and was appointed an Assistant Professor of Surgery at the Uniformed Services University of the Health Sciences.
She is excited to return to San Antonio and to provide comprehensive surgical care to our patients. In her free time she enjoys traveling, cooking, and spending time outdoors hiking, biking and running with her family.
